Interestingly enough, we have the same problem with our MOSS/K2 configuration.

Is there any known issue which could produce this kind of behaviour? What would prevent a worklist item to be created? Insufficient permission of the user who starts the workflow process? But if this is the reason, why K2 does not raise the flag about something going wrong?

I'm interested in the resolution of this issue.
