I have seen this issue sporadically in an environment where Workspace is load-balanced.

It seems to be due to Workspace calling out to SSRS, with affinity not set correctly. 
Thus, it isn't always the same NLB node hosting the Workspace session and the Report.

Do you have load-balanced K2?
